Complete this form to pick up your active prescription at Bridgercare's front desk. We require 1-2 business days to process your refill request. If someone else is picking up for you, please call our front desk to authorize them at 406-587-0681 ext. 0.
If you pick up your prescription at an outside pharmacy, please contact them directly for any refills or transfers.
Bridgercare is located at 1288 N 14th Avenue on the second floor.
